Log in to MyChart.\nFor records that aren't already available in MyChart – or if you don't use MyChart – you can request your records using any of the following methods you prefer. (For imaging requests, see the \"Radiology Imaging and Reports\" section below.)\n\n Request the records via MyChart: Go to Menu → Sharing Hub → Yourself → Request a copy.\n Submit a request online for UCSF Medical Center, UCSF Benioff Children's Hospital San Francisco, UCSF Benioff Children's Hospital Oakland or Langley Porter Psychiatric Hospital.\n Complete the health information release form and mail it to the address below. (Form for Spanish-speaking patients: Autorización de divulgación de información médica.)\n Mail us a written request with your medical record or unit number, your full name at the time of treatment, and your signature to authorize release of this information\n\nMail all written requests to:\nHealth Information Management Services\nUCSF Health\n400 Parnassus Ave., Room A88\nSan Francisco, CA 94143-0308\nWe'll process your request within 15 days and send you the records using the method you indicated. You also may arrange to pick up your medical records in person at any of our Medical Records locations. Our office hours are 8 a.m. to 4:30 p.m., Monday to Friday.\nTo make an appointment to review your medical records in our Medical Records office, call (415) 353-2221 during our office hours.\nProcessing fee \nWe charge 25¢ per page to copy medical records or $25 if using an electronic record. We'll call to notify you of the total charge. You may mail us a check or pay the fee in person if you pick up your records.\nRadiology imaging and reports\nYour imaging test will be interpreted by a UCSF radiologist, a doctor trained to interpret imaging exams. The radiologist will write a report with the results of your exam. After the report is finalized – typically in two to five working days – there are two different ways you can access it and receive your results: \n1. MyChart\nThe report and the images from your exam will be available in MyChart. This is the fastest way to receive your results. Please note: Because your results are available in MyChart immediately after they're finalized by the radiologist, your healthcare provider who ordered the study might not have reviewed them yet. \n2. UCSF Imaging Library\nThe report and images are also available by CD, free of charge. To request a CD, complete the Authorization for Release of Health Information form (English) or Autorización de divulgación de información médica (Spanish).\nPlease include the following when you submit your form: \n\n Type of imaging exam\n Date of imaging exam\n Mailing address (including zip code)\n\nEmail the form and this additional info to [email protected] or fax it to (415) 353-8583. \nCDs are mailed five to seven business days after the release form is received. \nIf you have any questions about obtaining a copy of your images and report, call the Radiology Imaging Library at (415) 353-1640 (option 3). Hours are 7 a.m. to 6 p.m. seven days a week. \nFor help understanding your images and report, contact the provider who placed your imaging order. \nHealth information exchange\nA health information exchange (HIE) allows your health care providers to use secure, electronic methods to share your most recent medical information with doctors' offices, hospitals, labs, radiology centers and other providers participating in your care. The purpose is to give each of your caregivers access to the most up-to-date information available.\nPatients are automatically set to opt in to HIE, meaning information will be shared with participating physicians and health care providers unless you provide instructions to opt out.
